Does it show up in your plugins list? sm plugins list
When you connect to the server do you download the models? Check your CSS folder, there should be a few files in cstrike/models/player/forward/

If you are using custom skins, those skins have to support a forward attachment. Installing this plugin will only make the default CSS skins work.
