1) Yes, but this only happens if you start it with its_enable 0, then turn it on. I dunno if I'm able to loop through all existing common infected and then remove them since they don't act as players, but I'll give it a try!
2) Only on maps that force spawn (such as the Dark Carnival finale)
3) The bridge finale uses a special finale start hook that I've been unable to find. So the finale is never actually started and as such any unintended tanks are killed. If someone finds the hook out I'll gladly add it

and the running timer is a known problem, I'm currently looking into alternatives!
