I'm not sure the author is still active either...try both versions 2.5a and 2.5b first...if both still create the door issue let me know. I'm working on a new version with a workaround for bill(model only select) and the rest are changeable into the characters as clones...leaving L4D1 setting on 1 in csm.cfg and playing on all maps L4D1 & L4D2 all characters are selectable, Bill is just a skin to prevent crashing...Only problem is character weapon models are getting screwed up when u switch, u don't see it but other players do...Simple fix is dropping the weapon and picking it up again, not sure how i'm gonna fix that yet...Anyway if u want maybe i can help you...I love this plugin and want it working again damnit...I will hopefully soon post my new version...
