1st, you should use the new thread over here
http://forums.alliedmods.net/showthread.php?p=729788 (and download the latest version from there) as this one has been retired.
The sm_time_between_sounds convar should work without problem, it has always worked for me, in every version of this plugin.
sm_sound_limit is per map.
In older versions of the plugin (including, I think, all of the versions in this thread) it did not have it's own config file and you would place the settings in the sourmod.cfg file. Later versions create a sm_saysounds.cfg file like you mentioned.
