...does crash happen EVERY map change ?
Have you tried, disable plugin and move plugin in "disabled" folder, one by one until crash stop ?

Not need disable SourceMod base plugins, only custom ones what you have installed.

My wild guess is, plugins which use "[12] PTaH (1.1.3): Additional CS:GO Hooks and Natives" extension could be a problem.
Have seen couple topics where PTaH is problem.
