Try this out, I used AtomicStryker's SM Respawn with his Ghost Everywhere (for teleport) and it seems to work well.

It's set right now so that you're just like the infected, if you're incapped you die and then respawn in X seconds.

Cvars:
l4d_survivorrespawn_enable (Default 1) - Turns this plugin on and off
l4d_survivorrespawn_timeout (Default 10) - How many seconds a Survivor must wait until they are respawned
