Get the latest snapshot from the stable branch for metamod and sourcemod [if you are using sourcemod]
http://www.metamodsource.net/snapshots - current latest is - 1.10.5-git937
http://www.sourcemod.net/snapshots.php - current latest is - 1.7.2-git5201

Once you have extracted these to the server then update the server one more time and see if the issue still happens.
