It appears you updated the plugin code to version 1.2 but missed this statement in the first post right above the file download...

The phrases file has been updated and is REQUIRED and must be copied to the "addons\sourcemod\translations" directory.
Just download the "l4d_ReverseBurn_and_ThrowableAnnouncer.phras es" file and copy it to the "addons\sourcemod\translations" directory and you should be good to go.
I also changed the text in that first message to make it clear version 1.2 has a new phrases file, so now it says...
