If you are using Left4DownTown be sure to update that, it has new signatures which will most likely be the cause of your issues.

1) Make sure of the same above
2) Make sure the boldened bit is written
3) you don't need to write the numbers in speech marks. I advise against it.
4) Post back if you still have troubles, my server has l4d_xtrahp 1 and it's running fine at present.

When this happens? I get the feeling it's not being set right.. I've been having troubles forcing it as of late.
