I did a test with [L4D & L4D2] Tank Rock Ignition plugin and it worked.

Probably is a conflict with [L4D & L4D2] Tank Rock Lag Compensation plugin, since it blocks damage being registered through OnTakeDamage, nothing that I can "fix" (thanks for reporting).
