Look ...addons/sourcemod/logs/ do you have any error logs. Show us.

When your server start up moment ago, use first server command:
sm_dump_handles myhandles1.txt

And when your server have run hour or so (and there are players, activity), use server command second time:
sm_dump_handles myhandles2.txt

and post both txt files here.
