usually they are connecting from cafe accounts which grab a random steam id from a huge bank of ids... in effect this gives them a new id every time they log in. its not that they are nosteam hacks, its just the way valve handles internet gaming cafe support... it sucks because this is the source of most of the cheating these days... an ipban works, but as most of these cafe's are in areas with limited ip space, they are almost all using NAT to connect... this means you lose that entire cafe when you ipban... its minor, but there may have been legit players there too that can no longer connect to you.
we will continue to have these issues until valve finds a better way to secure the cafe system... and we all know how "fast" valve moves on these types of issues... lol... this one has existed as long as css has been around... maybe longer...
