do you have +maxplayers in the command you start the server? You cant change it via console once the server is started.
In Valve's wiki +maxplayers is not valid for l4d2, but I feel it still works, and probably for servers with extra slots.
How many slots do you see via HLSW?
Maybe try to mess with sv_visiblemaxplayers?
