Try removing all unnecessary plugins, see if clients continue to crash, and if they don't, slowly add back plugins until you find out your root cause.

sm_rcon has a limit of information that it sends back to you in console. In staid you can use a tool like HLSW http://www.hlsw.org/, or you can use the built-in rcon function.
