Those files aren't even actual files needed. models and materials are supposed to be subfolders, not filenames. Yeah, something's not right.

Try disabling Sourcemod (and all plugins) by renaming the addons folder and see if it still happens.

Your plugins are pretty basic though, not sure why any of them would a crash on changemap.

There's also a line in sourcebans.cfg (addons/sourcemod/configs/sourcebans/sourcebans.cfg):

"DatabasePrefix" "sb"

Maybe check that? I think it never needs to be changed since both...

I don't think it's a connection issue since it seems like it found the database, but could not find the exact tables.

Maybe check the config.php in your Sourcebans install (should be on root...
