either you are not running that sourcemod plugin what create sm_cvar command (unknow command)

or you have in cfg file "new line" characters/symbols mixed...

Because there are too many players and big wide maps with all crap.

If your game is L4D, that "entityid" parameter not exist in event "infected_death"
And "infected_death" not get called if special zombie dies. (tested L4D2)

I didn't see that bug anymore. Only if you join server and pick class, some bot maybe do...

Now I see that weapons still are visible on real players....

hm... when they have removed null/world model :(, doesn't exist anymore.
Now it will crash players game when set modelindex to 0.

About SDKHook(ing), you should do this once on entity per sdkhook type.
UnHooking works automatically when entity not exist...
