Are you sure that the message to press escape doesn't appear when ingame? I've found that if a menu was open and you quit the menu will still be there, but non-functional.

And best map? That's debatable. Most use city 45 or city 18. I ran hometown for the fun of it and it worked out great.

Same exact bug on HL2DM. Mostly happens when stuff is spawned that wasn't precached correctly.
