I had the same issue and I reported a few months ago.

Using probability 5% I got 8 tanks in one map (limit 3). Sometimes double tanks, thats why I'm suggestion for spawn_interval.

I noticed a bug. Sometimes it spawns the wrong melee after you save and reload the map, I'd say 10% of them.

I'm facing the same issue. When is about to change campaing (with a plugin or changelevel manually) it crashes 50% of the times.

I have crashed valve servers (vanilla no sourcemod). This is an issue that came within the update.
