Is your server visible and does rcon work via the website (just as check) dont know if its needed, but in your sourcemod config folder, there is a sourcebans directory where the sourcebans config is....

Well it does say error connecting 173.208.165.228:27015, is that one correct. Does communication between the 2 work, is the rcon correct?

If you know the reason which you want to change for *all* then you can do a find / replace sort off. (in sql)

did you update your sourcemod and made a boo-boo by overwriting your databases.cfg?

or maybe the old "basebans.smx" is still loaded, and you forgot sourcebans.smx :P happened to me once..

Psy made the sourcebans checker, i would suggest making a alternation where it instead wars the current admins on the servers saying "this ip has a ban on record from blabla" rather then banning it...
