Sounds like you're using the original plugin. Try dirka's edits earlier in this thread. He increased the array size to 32. That should accommodate more players and fix the out of bounds error.

Just a response, turned out the auto setup had protected my sql suffix. http://www.cloudflare.com/wiki/What_subdomains_are_appropriate_for_orange_/_gray_clouds%3F

Turning it off fixed the issue.

cartman-2000 is correct, this error should only happen if your maxplayers is above the max array size set in the code. As far as I know L4D1 and L4D2 only support maxplayers of 18, so there shouldn't...
