Most likely caused by a custom plugin, or a custom module/addon. Usually custom modules/addons cause this. In that case, its not an AMXX issue.

Everything looks fine to me, try disabling all your custom plugins, and see if you have the same problem. If not, then enable each plugin one by one and see what one is preventing admin verification.
