Make sure what you are trying to spawn is a physics prop. If it isnt then you will need to use "prop_dynamic" for the classname instead.

Im going to guess that a concrete wall is a dynamic prop.

Sounds like sourcemod isnt running. Double check that both sourcemod and metamod are installed correctly.

Make sure the plugins are loaded. Easiest way to check is to use "sm plugins list" in console.

z_hunter_health 250 is showing what your client has the cvar set to, not the server. Dont know why sm_cvar would not be working for you though.

This plugin is horrible.

It rebinds the mousewheel to say a chat command and overwrites whatever they had bound to there mousewheel, most likely weapon switching.

If im not mistaken, using z_difficulty doesnt actually disallow lobbies of other difficulties from connecting, it only forces them to a difficulty.

The only solution i know is to remove that and...
