I think you're on the wrong axis, it looks 90 degrees off - add the loop so you're drawing all the lines and it'll probably be obvious.

Using the entity index directly in your case should be fine (if you only need to handle networkable entities), if you want to be extra safe, store the entref as the first block for each array item...

There should be no measurable performance impact, it only really does any work on map change then it just sits waiting for a crash.
