The database is only accessed when map load.
Basically what happening is that the file addons/sourcemod/configs/admins.cfg is updated during mapchange. And the players are checked against that file...

I dont know, I only use the menu, but I assume the command and the menu will work the same way.

Nope, not 100%!

Still getting an error when banning in-game via the menu thats says it doesn't work, but the player with steamid will end up in the database.

Add the ip/port of the gameserver into the cfg-file (addons/sourcemod/configs/sourcebans/sourcebans.cfg)

This is obviously how the server is identified and is required even if you dont use...
