Do you have two users, both squaredo_sb and squaredo? Because the above line requires a username before the @ symbol.

I don't use PHPMyAdmin, I just use Linux command lines. I haven't used the former before. There are a few pages explaining how to do it for PHPMyAdmin.

Then you haven't configured PHP to use MySQL.

Open /etc/php/php.ini and uncomment the line:

Upon further inspection, SourceBans' style sheets are provided by PHP. You probably don't have PHP enabled on your server.
