Your vault may be corrupt. Have you tried shutting down the server and deleting the vault file and then restarting the server?

If you are running AMX-X 1.8.2 or an older 1.8.3 nightly build then you also have to replace your nvault dll with the one in the nvault utility thread or you will get errors.
