Yes. This is the same problem we are having. People will have the tag when they join the server, but when the map changes, someone randomly loses the country code.
We tried to hook different events...

Yes, same problem is happening to me sometimes. The teamkiller gets banned, but is not kicked. They can continue playing until they either are voted out or leave.

This happens with Insurgency 2014. the Unicode Name Filter (https://forums.alliedmods.net/showthread.php?p=2207177) does change the name properly, but it does not support scandinavian alphabets...
