Unable to login using my sb login. Just says "failed login attempt" in the logs and "wrong username or password". I've got the SB salt fix and tried a second installation and it still wont work.

you need to edit the mysql config /etc/my.cnf via ssh using nano or vim and add this line to it.

bind-address=173.254.194.142 save and restart the mysql service in cpanel.

If your server does use Banned_user.cfg for storing all Perm bans you need to wait for mapchage or reboot the server. If you are using some 3rd party mod for banning i.e sourcebans etc you don't need...

it would be in your cfg folder, file name will be banned_user.cfg and you would add it like this

banid 0 STEAM_0:0:25652720

that steamid is just an example.

This plugin respawns you once in mg_saw_iv then it stops respawning players
