Did you disable the plugin by dragging it to the disabled folder and restarting your server, or using "sm plugins refresh"? Were all 8 survivors human players or were some of them bots? It's possible...

Add sm_cvar l4d2_hx_health "0" to your server.cfg/listenserver.cfg file. See my guide for more tips.
