You don't even need to include the port, the IP is all that is required

There is no reason to use other MasterServers, just leave the file alone and don't touch it. As long as you are using the latest version from Steam of both the Client and the Server, everything will...

If you are getting "Bad Request" then you are formatting it incorrectly. As long as your server is installed using SteamCMD and your ports are properly forwarded, it will be listed on the Master...

Simply replace the <SERVER IP HERE> part with your server IP and it will output a list of all server reporting on the Master Servers on that IP.
