Modules Error Everytime!

Well, I recently posted a thread on modules errors on my server. I got some great help from Anti and it worked fine for a couple hours and bam. Same error, Keep in mind this is in my IA server:

1. I tried my best followed everything. The rest of the servers I setup are fine.

I actually found a tech who installed these things, he does not get the problem either. All the plugins seem to be running fine, I haven't added any plugins recently to CTF nor IA. IA never...

So yea, I just solved my map problem and now everytime I start the server it starts and goes down within the next hour with this error:

Yes I did a fresh install without even touching the server.cfg now im only getting

./hlds_run: line 321: 16152 Segmentation fault $HL_CMD


and after 2-5 mins it says server quit
