I think you're on the wrong axis, it looks 90 degrees off - add the loop so you're drawing all the lines and it'll probably be obvious.

PTaH's hook cannot remove DataTable warnings as they're generated by the networking thread...

Using the entity index directly in your case should be fine (if you only need to handle networkable entities), if you want to be extra safe, store the entref as the first block for each array item...

There should be no measurable performance impact, it only really does any work on map change then it just sits waiting for a crash.

The error message you have posted indicates that "localhost" has been set at the "host" in the databases config.

Use "127.0.0.1" or the correct path to the socket file instead of "localhost" as the DB hostname.

If queries or web requests are involved, it could take any amount of time.

SM doesn't know when every plugin has finished making their admin cache changes, so it can't notify plugins of that state.

I highly doubt it, it is gamedata heavy and thus tied to a specific release TF2 (the latest one), which is not your compile of the TF2 game code on the sdk2013 engine, and definitely not your compile...
