Sometimes when players press the use key on an ammo pile, ragdoll, switch or movable prop, they enter the T-pose and get dragged into the object. Ammo doesn't get refilled, switches don't activate...

Made a quick modification to McFlurry's version. All players can spawn weapons, but zombies and ammo count are still admin only.

Well, I fixed it by getting rid of L4DToolz and using Left4Downtown for 4+ player co-op instead. Whatever works.

I've gotten it to work in L4D2. The catch is: you have to manually send the cached sound files to your players in addon format (VPK). Not a problem for a small friends server.

Another bit of info... only the new hats still show up. Within the last two content releases, to be more specific. It's my suspicion that an item/backpack tracking system is the culprit.
