Are you running Accelerator? do you know how to view the automatically generated core files yourself? can you attach gdb? can you start your server with gdb? can you upload your debug.log file?

You should use Accelerator since you're not developing.

Do you have a line number? Which version of SM is this? is this a custom SM build? Can you grab the .dbg from your build for logic bin?

Can you print the values of at least eip and probably eax?

As a hack around crashing, hook an entity of the same typw outside of the callback.

There's at least a 0.1s delay.

It sounds like you know exactly what's going wrong with DHooks. If you can glue this to stock SourceMod 1.6, I would be more then happy to spend hours to fix this...
