The sma file attached in an above post by VINAGHOST is only for testing some stuff out and is not officially for "Public Use". Discard the file.

You should always download plugin sources...

Ah, good catch. Can't believe I missed that myself. I'm happy it's all good for you now. Reason why it worked for me is because my table is 'name' and so that one really went unnoticed.

So, your HLDS and Database is on same server.
And, Website on a different server. Right?

Then go to your ~/amxmodx/configs/sql.cfg and set
amx_sql_host "127.0.0.1"
amx_sql_type "mysql"

Hi,

It’s because the ban record somehow doesn’t get written in the advanced_bans.txt data file.
Head over to ~/addons/amxmodx/data, check if advanced_bans.txt actually has the ban written in it....
