I put them in server.cfg, but you can put them in any config file that is loaded.

The plugin works without any config. You only need to set a cvar if you don't like its default.

mp_match_end_at_timelimit 1

otherwise when the time reaches 0:00 it waits for the end of the current round (which won't happen if the surf map is unwinnable).
