You approached it correctly, however the .sp files are the source files. You will need to recompile the plugin.

Not lazy, there is no use for a world model with the purpose it was released for. There is no situation where a world model would be used for this knife, and they certainly are not supporting...

No options for non-premium/vip usage restriction will be added. Do not request it, do it yourself if you want to.

They do get banned. You won't get support for that on this forum.

Correct. If you don't use a GSLT, your server also will not be able to be connected to, unless you're running sv_lan 1.

If by "XXXX" you mean it is your GSLT, then no, you will still get banned.

I don't believe sv_lan 1 requires a GSLT.
