Well there. Its been forever, but the plugin is updated to not error out when people leave server on multi.
We've tested this plugin and its quite fun. :)

CVAR to decrease Parish Bridge spawns.

We play a realism Co-op mod on my server and the extra challenge calls for much less zombies.

I use these Cvars to reduce common spawns:

sm_cvar z_mega_mob_size
sm_cvar z_common_limit ...

as do I. There is really only one way to do that and that is adding

sm_cvar sv_disable_glow_survivors 1

to your server config.

However? why have you limited it to Realism? I actually use it on Campaign on my server.

Download mcfluries Plugin (the source).
Open it with:

Replace all instances of "hidehud 64" with "hidehud 345"

Then "compile" the...

I'm not a fan of 345 as it hides the crosshair. I dont think its very realistic because there are no iron sights in L4D.
I think it also disables weapon select and crashes client if...

Glow is easy to remove with Cvars.

Hidehud 64 however gives me issues.

I wan't a way to effect all players HUDs when they join my server.

I want a Cvar or plugin to emulate the Iron man Mutation.

If someone dies, they are dead. If the team dies, you start at the first stage of the campaign.

Is there something like this out there?

Sniper Rifles still dont kill in one hit all the time. CI take 2 chest shots on...
