Quick update:

Balance should work now, I removed the ratio cvar, it'll just balance equally.
You also can't swap teams once someone has been killed.
Suicides are ignored.

Hmmm. Could you somehow check the delta distance of each player?
Obviously when teleporting there distance is gonna change loads more than they could move. So if there delta distance is > whatever...

Yes it should be turned off.
However, this is done automatically when you active the plugin.
I think the problem is that your activating it after the map is loaded.
