I would disable the plugins one by one to check if I could successfully replicate this, but it seems to happen very rarely and I'm not sure why. More a curiosity than anything.

where did you put sv_cheats 1? it needs to be in server.cfg i think if you want it constantly enabled. not sure if you can type amx_rcon sv_cheats 1 in console and have it change instantly but i bet...

Move admin_slash to the bottom of the list in plugins.ini it makes things like
"/rank" go through as amx_rank, it's a crazy useful plugin for lazy people and pretty much my favorite but it's gotta...

Pretty sure you have amx_super or something that contains the bullet_damage plugin.

To turn that part off you can put bullet_damage 0 in server.cfg and restart. It should be gone forever.
