Dunno, may be related to the bot takeover event (bot being replaced by player). but is just a guess
What I do in these cases is having a global var that check if the hook event was already...

quick fix workaround is to remove the Tank skin from your local server,
since according to you is just one skin causing it, I would remove it
Try setting sv_pure and sv_consistency to 0 as well,...

I have no experience with this extension.
Another workaround is trying to use the CommandABot VScript command (BOT_CMD_ATTACK enum).
I believe left4dhooks nowadays has a native for this

It's because you are using a client filter
"filtername" "sky_filter_survs"
this probably only applies to survivors, try setting it to empty
