Did you place the .smx file into the "Plugins" folder in the sourcemod directory or the root directory of Sourcemod? Also I dont know for sure but you may need to have other players on the infected...

It seems to work now on Day Of Defeat Source. I did the same test I did last night and no problems after changing maps. I even set it up on Team Fortress 2 to see if works there...
