In what cases do players get banned automatically, and how do u set the settings for automatic bans/no bans on players?

Where are the files for the settings located...?
(I'm sure about the ban duration and all, but basically how do i find which events players get banned on and how can I set my own settings for automatic bans?)

Where are ban records stored? Which file(s)
I understand that KAC, SMAC, and other might have different cfg files/locations....but where???
