Sourceban setup error

Im trying to setup sourceban. Problem is when i upload the website folder to my website and try to go to the website.com/sourceban, then i get this:

The website main page works and so does anything else. Its only sourceban that gives me that error.

Your htaccess is bad probably, remove it for test.

That worked thank you! Now i just get this error instead when i try install donations. /donations/admin and i get this Unable to open connection to MySQL server.

my error log shows me:

[sourcebans.smx] Failed to query database. (Cannot add or update a child row: a foreign key constraint fails (`csgoserver`.`sb_actions`, CONSTRAINT `action_server` FOREIGN KEY (`server_id`) REFERENCES `sb_servers` (`id`) ON DELETE CASCADE))
