My Sourcebans++ has errors on the website

Hello guys, For now I'm sorry for my bad English, but I'm here to see if someone could help me with a SourceBans plugin that isn't working on my site. After I complete the second installment on the web site he gives me this error (http://prntscr.com/r5s557) could anyone help ?? Thanks to those who did not ignore !!
