Change client convar
 

How can I change a command such as weapon_debug_spread_show 2 with a extension?

The reason I cannot change that cvar using SourceMod scripting is that the command is not in the list of changeable client-sided convars.

Extensions don't have any greater ability to cross the client-server divide than plugins do.
