Sourcebans cannot see server online.

Like the title says. My sourcebans can't see my server online. I downloaded the test script and this is what im getting:
(MY-SERVER-IP is insted of typingmy ip address)

[+] Trying to establish UDP connection
[+] UDP connection successfull!
[+] Trying to write to the socket
[+] Successfully requested server info. (That doesn't mean anything on an UDP stream.) Reading...
[-] Error getting server info. Can't read from UDP stream. Port is possibly blocked.

[+] Trying to establish TCP connection
[-] Error connecting #10060: A connection attempt failed because the connected party did not properly respond after a period of time, or established connection failed because connected host has failed to respond.

My firewall/router name is Dlink DFL-210

Simple ports are blocked and you need them open

Yeah i know but they are open. 27015 are open in both UDP and TCP.

What about on the web host? Are the outbound TCP and UDP ports open there?

But in my sourcebans list serwer http://sb.minigun.pl/index.php?p=home show connect error.

Any sugestion ?
