the error probably lies somewhere in your serverlist. do you have the same list on all servers? especially: do you have the same number of servers in all lists on all servers? and if you got more than 5 servers: did you remember to recompile the plugin for all servers with #define max_serverforwards to something higher than 5?

the issue occurs when you got e.g. 3 servers in your list and a player connects to the server carrying the information "i am coming from server 4". as there are only 3 servers in the list it tries to access an index in the list that isn't there ("index out of bounds").

it could also be caused by a player who had this information in his config and then made his config file read-only. check which player connected at the time you got these errors and see whether it's always the same person.
