[TF2] FistPie - Prevents server crash exploit

The exploit that this fixes:

Disguise as friendly heavy with fists (NOT KGB)
Get shot with dead ringer out
Uncloak immediately and/or feign death (?)
Server goes poof (most of the time)

Tested and confirmed working to prevent anyone from disguising as a friendly heavy (hardly a gameplay breaker). Optionally (disabled by default) bans repeated offenders for 30 minutes.

0.4:

Banning moved to compile flag and disabled by default

0.3:

Bans for 30 minutes on third strike.
Altered message.
Added logging for name and steam id.
Fixed handle leak

I've used it several times on a variety of servers, but maybe the halloween update fixed it :/

Also I believe there needs to be at least 24 people playing for it to work(?)

It's not fixed, but it only works on some servers.

Thanks so much for this, my server was getting hit a lot, and yes its still possible after the halloween update.

I was testing the exploit on my server, it doesn't seem to work, although I might not be fast enough uncloaking.
(People just wont shoot you when you expect them too !)

Anyway, I think I'll install this just to be safe.
