I didn't want to put together a hackjob vs only server and was tired of two player coops being held on my server. This solved the problem nicely.

Well, ran into the same problem with the blank record. I checked the logs last night and I didn't see anything that matched up that would suggest a sql insertion problem.

In my situation the record wasn't totally blank. I think it just didn't have the name. Next time this pops up I will double check it but I know there was data in the record.

The collation on the players table is for the three fields

SteamID
Name
Lastontime

Each of those is 'latin1_swedish_ci'

I will have to check the log a little later.

Dropping that record worked but some reason there are people on my server that isn't being shown by players online now. Game-monitor.com shows them but the players online doesn't.
